Project Assistant-Health Services (PA-HS) (2 positions)

Area of Operation: Kisumu West Sub County- Kisumu County.

Overall Purpose: PA-HS will work under the supervision of HSDO. He/ She will also work with a team of case managers at site offices to capacity build Case Workers (CHVs), Link Desk Persons (LDPs), CALHIV and HIV positive caregivers on HIV education. The PA –HS will be expected to manage a team of LDPs in CALHIV case identification-(CALHIV, PMTCT and HEI), enrolment and health related service delivery to the said beneficiaries.

Specific Responsibilities:

  • With support from HSDO conduct CALHIV co-creation activates to inform and improve OVC programing.
  • Spearhead formation and facilitation of OTZ Clubs/PSSGs
  • With the help of Case managers and LDPs, spearhead scheduling and participation in MDTs, and Case conferencing for HIV positive OVC suspected to be failing treatment.
  • With support from case managers, characterize and formulate joint suppression plans with the care and treatment partners for HIV positive OVC suspected to be failing treatment
  • Participate in CME and facility data review meetings.
  • With the help of Case managers, case workers and LDP, spearhead follow up and share information on LTF CALHIV at facility.
  • Work together with case Managers to ensure targeting of CALHIV services is done.
  • Work with the Case Mangers and spearhead HIV/AIDS S&D campaigns within the coverage areas.
  • Verifying CALHIV data at the facility and ensuring entries on CALHIV trackers are accurate and up to date. Ensure CALHIV data from the facilities capture defined variables.
  • Coordinates with the Case Managers and DA’s in ensuring that required data is timely sent to HSDO and project MEL person.
  • Follow up CALHIV referrals for treatment at facility, spearhead bill waivers for the needy HHs.
  • Work with Case managers, Case workers and LDPs to ensure daily filing of HIV information in CALHIV files-including case plan updates and changes.
  • Work with the Case Managers and LDPs to track and maintain Bi-directional referral registers (referral inn and referral out) for CALHIV, other OVC and HIV positive caregivers at the site offices.
  • Ensure the Case workers and LDPs understand all the SOPs on CALHIV and related services
  • Track and enroll children of KEY POPULATION within the coverage areas.
  • With the help of Case Managers, Track, Monitor and document CALHIV HH performance on Healthy domain under case management.
  • Document monthly success stories on CALHIV for replication
  • Work with QITs at LAAC to inject quality in provision of health services.

Required qualifications and experience:

  • Diploma in Nursing, Community health, Nutrition, public health and other relevant academic qualifications.
  • Candidates with practical experience (3 years plus) working in health facility setting (medium or high volume) in PMTCT, MCH or CCC clinics will have an added advantage.

Knowledge, Skills and Abilities:

  • Willingness and flexibility to work long hours
  • Proficiency in written and spoken English and Swahili.
  • Excellent knowledge of Microsoft Office;
  • Strong organizational and interpersonal skills and ability to work in a team-oriented setting;
  • Valid Motor cycle Riding licence will be an added advantage.
  • Ability to speak local language will be and added advantage.
